CareZone Emergency Card

The CareZone Emergency Card is an easy-to-read, wallet-sized card with important info needed during an emergency, or even a clinic visit, that you can print right from the app. It includes your full name, date of birth, allergies, health conditions, and the medicines you're currently taking.


iPhone and iPad Instructions

Editing information on the Emergency Card

  1. Tap on the profile name ("You", "Dad") at the top of the module list. If you don't see it, tap "<" in the upper left corner.
  2. Tap Edit in the upper right hand corner
  3. Tap on the section that you'd like to update, such as date of birth or allergies.
  4. Tap Save on the top right of the page.

Printing the card

  1. Go to the Medications module.
  2. Tap on the share button in the lower right corner (looks like a box with an arrow).
  3. Swipe right to left and tap to select Wallet Size
  4. Tap on Share in the upper right corner.
  5. Tap Print.

Android Instructions

Editing information on the Emergency Card

  1. Tap the three dashed lines in the upper left corner
  2. Tap Home.
  3. Tap View Profile.
  4. Tap Complete this profile.
  5. Tap on the Pencil button in the lower right hand corner to edit your profile
  6. Tap on a field to enter information.
  7. Tap the checkmark at the top to save your changes. 

Printing the card

  1. Go to the Medications module.
  2. Tap on the share button in the upper right corner (looks like a <: ).
  3. Select Wallet.
  4. Tap on the printer icon in the upper right corner.
  5. Tap Print.
